Project Description |
The project will aim at enrolling up to 25,000 individuals in the Brisbane and Ipswich region, supporting hospital to GP and GP to hospital communications for specific patient cohorts. There are four key areas where the use of IHIs to support electronic exchange of information will be deployed:
